PLEASE READ INSTRUCTIONS PRIOR TO BEGINNING APPLICATION
Renewal application must be completed in one session. Please have the following information / documents available prior to beginning application:
- License number
- Last four digits of Social Security number
- Continuing education information
- Signed Supervisory Statement (COTA's employed in WV only)
- Name change legal document if applicable
- Credit card information
You will begin by entering your license #, date of birth, and last four digits of your social security number on the login screen.
Confirm the Applicant Information is correct. Name and address changes may be made at this time. The legal document authorizing a name change must be uploaded if you have not previously provided this information to the Board.
Complete the entire application. Application must be completed in one session.
After entering employer information, you must click "Add Facility", even if you have only one employer. Multiple facilities or agencies may be added.
All OTA's employed in WV are required to upload a new Supervisory Statement as part of your renewal application. Up to three supervisory statements may be uploaded. You must enter employment information to receive the prompt to upload a Supervisory Statement. Applications received without a Supervisory Statement will be rejected pending receipt of a signed supervisory statement.
In the Continuing Education section of the application, please choose the applicable type of CE by using the dropdown boxes. After entering each CE activity, please click "Add". The application will calculate your total CE hours entered.
Licensees who have been selected for the random CE audit are able to upload their Continuing Education documents.
Please review the summary provided prior to clicking "Continue".
Enter your credit card information on the State Treasurer's Office secure payment page. Upon approval, you will receive a receipt to print, as well as the opportunity to export a copy of your renewal, for your records.
You must click "Continue" on the receipt page for the renewal process to be completed.
Application must be submitted by December 31, 2017 to avoid assessment of $100 late fee.
Your new license cards will be mailed to you and should be received within two weeks.
If you prefer to use a paper application, please complete the application you received in the mail or print the appropriate application below, and mail to the Board with a check or money order for the appropriate fee. Please use regular US Postal Service to avoid any delay in delivery due to a signature requirement. Applications must be postmarked by 12/31/17 to avoid a late fee.
