NAME CHANGE:
To change your legal name with the Board and request new license cards, please log in to your licensee account using the Log In link below. Once logged in, hover over your name at the top right and select "Profile Details". Then select "Name Change" and enter your new name.
You must upload a copy of the legal document authorizing the change. Acceptable legal documents include a marriage certificate, divorce decree, or other court document ordering a name change.
*A new driver's license or social security card ARE NOT acceptable documents and will be rejected.
Upon receipt and approval by the Board, new license cards will be mailed.
ADDRESS CHANGE:
To change your mailing address with the Board, please log in to your licensee account using the Log In link below. Once logged in, hover over your name at the top right and select "Profile Details". Then select "Profile Fields" and enter your new address.
